Ĉi tiu regado ŝajnas kontraŭdire. Ĉiuj diras. “neniam uzas la akuzativon malantaŭ prepozicioj!!!” , sed JEN! Nun tiu regado ne vere ĝustas iafoje.

"neniam uzu akazativon post ORDINARAJ prepozicioj.
Se vi uzas akuzativon post LOKA prepozicio, ĝi signifas direkton.
"al mi" ne estas akuzativo, ĝi estas dativo, sed en via frazo ĝi signifas "mia".
Kial oni diras "al mi" anstataŭ "mia"? Ĉar "mia" estas dusilaba vorto kaj ĉiam havas akcenton je la du silabo de la fino.
"al mi" estas du unusilabaj vortetoj, kiu restas senakcentaj (klitikoj) kaj kiuj oni prononcas kiel unu vorto kun antaŭa aŭ posta vorto kun akcento. "al mi la piEdon" vi povas prononci kiel unu vorto ("la" ankaŭ estas klitiko), sed "mIa | piEdo" estas du vortoj.
"mi dolorIgis | al mi la piEdon" - "l hurted | my foot"
En la frazo de StefKo:
"mI al mi | dolorIgis | la piEdon" ≈ "I to myself hurted the foot" - la sama signifo, sed kun akcento al subjekto.

The sentence

      Mi dolorigis al mi la piedon

means (as Sergejm said in the very beginning)

      Mi faris tiel, ke mia piedo ekdoloris

with the idea, that I caused the pain myself (e.g. I hit a chair), but that's not the point, the pain in my foot is.

Now you wonder what this "al mi" in the original sentence is. Let me explain. For some reason many E-o speakers prefer the romance language way when it comes to bodyparts, the foot instead of my foot, the head instead of my head and so on. Allegedly the logic goes like this: "the" is enough because it is obvious to whom the bodypart belongs to (I disagree with this "obviousness"). So you could write the sentence into form

      Mi dolorigis la piedon

since it's "obvious" whose foot got hurt. Or you could use the possessive pronoun (I prefer this way)

      Mi dolorigis mian piedon

In the original sentence the writer has wanted to use the "the" form but to clarify, whose bodypart it is. The result is a sentence

      (Mi) (dolorigis) (al mi) (la piedon)

      (I) (caused pain) (to me) (the foot)

where "al mi" is in what is called the dative case (the receiver of an action). In order to get fluent English you have to edit (I) (caused pain) (to me) (the foot) a little, e.g. My foot got hurt (or something like that, I'm not a native speaker of English).

It takes a little time to learn how to formulate search queries, but it's well worth the investment.

The search query to see how dolorigi is used is backslash b dolorig backslash capital V capital F.

This reveals that the most common usage is of the form dolorigi followed by the accusative rather than dolorigi al pronoun accusative.

So the accusative is no more puzzling than it is in mi trinkas kafon or mi riparigis la aŭton or mi diris al li la veron

It's the same as "Mi dolorigis mian piedon" but there's a history in Esperanto of using Romance constructions such as "mi brosas al mi la harojn" in place of "... miajn harojn". That's why we have an example of the construction in the book. You don't have to use it though.
