Yes, I have. It can be found HERE.
I've been using it for almost a year now, and it works fine. I use it on both XP and Vista.
It is basically the English QWERTY layout, with the non-Esperanto characters replaced by the Esperanto characters, and a few punctuation marks rearranged.
