I'm a little confused on the differences between, and proper use of, ekster and ekstere. The definitions make me think that they can both mean "out" or "outside," but are they really synonymous? Or what are the nuances here that I'm missing?

Ekster is a preposition and ekstere an adverb. Remember that prepositions always require a main word after them in Esperanto. If you do not have the main word, you use the adverb. Since both ekster and ekstere have with a location to do, you can use accusative to show motion.

  • Mi staras ekster la domo : I am standing outside the house.
  • Ni devas elsendi iun ekster la tendaron : We must send someone out of the camp.
  • Starante ekstere li povis vidi nur… : (While) standing outside he could only see…
  • Ŝi volis iri eksteren : She wanted to go out(side).

Okay, so it sounds like if you use "ekster" then you need to specify what it is that you're talking about being outside of or out of, is that correct?
