Almost a) these days. The -ly is loosing steam.
You could even say you want the car real bad. (Although this example suggests a speaker a little farther south then I, it is certainly acceptable and would raise no eyebrows.)

I'm on the North East Coast of North America, but I think its a widespread construction.

It's not correct, but it is unfortunately common. As vejktoro says, you can even drop the -ly.

I live in the Portland, Oregon area. You would not sound out of place in many areas around here, saying things that way. I've said things like that myself (although not in a formal setting).

I'd place it as semi-rural, small-town American English. But you'll hear it in the cities as well; just maybe not as much.

No, I wouldn't. Penige means something different to real bad. Perhaps urĝe...urĝege would do the job.

I would probably say "Mi treege volas aŭton" or "Mi vere volas aŭton".
